Tioga County Case Manager & Residential Assistant
Catholic Charities Tompkins/Tioga has a job opening for a Case Manager/Residential Assistant in Tioga County. This position supports a transitional residential facility and assists individuals with mental health to obtain independent housing and provides supportive case management.
Position Title: Case Manager/Resident Assistant - Tioga
Salary: Starting at $21.50-23.00/hour depending on relevant years of experience.
Schedule: Full Time, flexible hours. Tuesday – Saturday 10:00am – 6:00pm.

SPECIFIC DUTIES:
· Conducting assessments & services, serving clients referred by ACT/SOS team or partnering agencies.
o Guiding clients to complete applications for financial assistance, finding the best fit among community funders based on the clients’ needs, goals & abilities.
o Encourage clients to access other resources as needed.
o Referring clients to appropriate and relevant services outside of CCTT
· Maintains a caseload of clients and maintains their case files in accordance with Agency, departmental, regulator and funder guidelines.
o Completes relevant screenings and assessments such as but not limited to psychosocial, mental health, financial status, and employment status.
o Creates, implements, and monitors comprehensive safe care plans and goals.
o Visits Clients at treatment center, program site, home or work site as appropriate to the individual plan of services to accomplish stated duties, assist the client in stabilization and engagement in treatment and other needed services.
o Assists in obtaining support for clients to include but not limited to home care, medical, dental, legal, Department of Social Services (DSS), Child Protective Services (CPS), and clothing and food needs.
o Assures communication and coordination of health and mental health care.
o Accompanies individuals to appointments as required.
o Assist the client in understanding and meeting expectations of service providers, courts, landlords, and employers.
o Inform clients of opportunities for social support such as recreation, faith based, or self-help involvement.
o May deliver instructions in various topics that support client’s ability to live independently.
§ Life skills facilitation and or coordination
o Conduct meetings with friends and family of clients to help with their understanding of how to assist in meeting client needs and the delivery of their care.
· Collaborates with referring agencies, funders and all related internal staff to effectively support individual client needs.
o Maintains regular contact to coordinate client benefits and address all other treatment needs.
o Reports to service providers about significant individual behavior or events that will require an immediate response.
o Acquire concrete resources to meet individual needs.
o Complete letters of requests/referral forms, applications, and other required documents of eligibility or charitable organizations and public agencies to advocate for resources that the client is entitled or eligible for.
o Makes referrals, education, employment, and civil legal issues regarding entitlements, eviction, credit and debt, custody and visitation, child support enforcement or modifications, and orders of protection.
o Document need for financial assistance or other resources such as wrap around from the project to stabilize client finances.
o Utilizing various databases and/or on-line platforms, maintains all documentation in accord with the agency and appropriate regulatory standards.
· Participates in relevant Agency meetings and ongoing development of professional knowledge and skills.
o Actively supports the financial stability of the organization by meeting or exceeding established productivity and documentation standards.
o Informs supervisor of significant events / issues related to clients as appropriate and follows program’s defined incident reporting process as required.
o Maintains and develops professional knowledge by completing recommended education and training, workshops, conference attendance, and readings.
o Maintains knowledge of regulations and standards that govern client population.
o Participates in appropriate interagency meetings with other treatment agencies and community groups as required. Provide updates and communication with providers including those who cannot be represented in person.
· Encourages individuals to be involved in daily activities, observe behavior and ensures the maintenance of appropriate conduct.
· Provides training in living skills through individual and group work.
· Provides safety and integrity of the facility at all times.
· Ensures the adherence of confidentiality.
· Works on the goals that have been established between individual and Director.
· Writes case notes and take required data based on goals.
· Develops and hold accountable a chores list for all individuals in the house.
· Reviews and hold accountable all policies and procedures of house rules. Communicate to Director any rules not adhered to.
· Coordinates services such as make referrals, help arrange transportation to places, possibly attend appointments with individuals.
· Ability to work independently in a community setting informing supervisor of concerns.
· Maintains professional practices.
· Communicate effectively and works cooperatively with individuals in the house, staff and community professionals.
· Awareness of and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.
· Adheres to policies and procedures established by Catholic Charities of Tompkins/Tioga.
· Other duties as assigned
QUALIFICATIONS:
· High School Diploma or Equivalent, Associates Degree Preferred.
· 2 years’ experience working in Housing and/or Mental Health
· Familiarity working with individuals with substance use disorder.
· Strong verbal and written communication skills, with ability to multitask, and strong attention to detail.
· Valid NYS Driver’s License and access to a reliable vehicle.
· First Aid, CPR, and Narcan training within 90 days of hire, and recertification every 2 years as needed.

Our hiring process and timeline:
