Organization Overview: The Source LGBT+ Center is a prominent LGBTQ+ community center serving the Central Valley of California. We provide essential support services, resources, and educational programs to empower and uplift People Living With HIV (PLWH), youth, LGBTQ+ individuals, their families, and allies.
Position Summary: The Source LGBT+ Center’s Case Manager/PrEP Navigator is responsible for coordinating a range of client-centered services for people living with HIV and those seeking PrEP/PEP. They are responsible for advocating for clients, completing client care plans, annual reassessments to ensure eligibility, and will provide client-centered PrEP and PEP education, engagement, patient navigation and adherence counseling to PrEP and PEP patients to ensure patient engagement and retention in PrEP services. The Case Manager/PrEP Navigator is a key external representative of The Source LGBT+ Center. This position reports to and is under the direction of the Clinical Manager.
Pay and Benefits:
The Source LGBT+ Center offers a competitive compensation and benefits package. $27 to $34.04 an hour (annualized $56,160-$70,803), DOE. Responsibilities:
- Mission & Leadership
- Work collaboratively with the Clinical Manager, Case Manager, and all staff to ensure The Source carries out its mission, vision, strategic plan and Theory of Change.
- Cultivate and sustain relationships with vendors and community partners with the goal of ensuring sufficient resources and access to services.
- Represent The Source in external relations by serving as the organization’s official representative on select bodies.
- Team Management & Supervision
- Work with staff to develop objective measurements across all programs, to ensure consistent, high-quality evaluation and services implementation.
- Instill a sense of accountability among team members.
- Participate in the recruitment, hiring, training and orientation of program staff members.
- Client Services and Program Implementation
- Assist in the development and implementation of HIV & Prevention Services.
- New client intakes and annual recertification to determine program eligibility.
- Ongoing case management, benefits navigation, and transportation coordination.
- Distribution and documentation of food vouchers, prescriptions, and incentives to eligible clients.
- Coordinate services for clients and provide linkage to care for HIV/STI preventative and treatment services. PrEP/PEP Navigation.
- Assist clients in accessing other outside services and maintaining eligibility.
- Provide rapid HIV/HCV/Syphilis antibody testing and post-test counseling.
- Build and maintain rapport with clients.
- Conduct training to educate and advocate for the use of PrEP/PEP to medical providers, service providers, and clients. Onsite and offsite, traveling required.
- Participate in outreach efforts at health fairs, community events, clinics, The Source LGBT+ Center, etc.
- Conduct presentations to educate about STI's/HIV and at-risk behaviors to promote protection, prevention methods, risk/harm reduction, and LGBT sex education. Onsite and offsite, traveling required.
- Evaluate and seek feedback from clients.
- Administrative Oversight
- Maintain accurate records for grant reporting.
- Maintain HIPPA-compliant patient files ensuring patient confidentiality.
- Responsible for timely data collection, maintenance, and oversight of sensitive client information including both physical and electronic health records systems.
- Special Events
- Participate in organizational and department events as needed.
Qualifications:
The Case Manager/PrEP Navigator will be committed to The Source’s mission. Specific requirements include:
- Bilingual English-Spanish desired.
- Knowledge of HIV/AIDS, PrEP/PEP, community resources, health education methods, advocacy, harm reduction and general clinical knowledge.
- Advanced degree, specialty certification (CNA/MA/etc), and/or work experience equivalent required.
- Reliable transportation, a valid CA drivers license, a safe driving record, and vehicle insurance as use of personal vehicle is required.
- HIV/HCV Test Counselor Certified (BCST) preferred, otherwise will be trained.
- Previous experience with Ryan White Programs, ADAP, HOPWA, LEO and/or ARIES highly desired.
- Proven experience providing case management/direct services to clients and/or patients preferred.
- A professional and resourceful style; the ability to work independently and as a team player, to take initiative, and to manage multiple tasks and projects at a time.
- Excellent communication skills, both written and oral.
- Excellent time management, organizational skills, and minute detail-oriented is crucial.
- Tech savvy, competent in MS Office, Google G Suite.
- Ability to work effectively in collaboration with diverse groups of people.
- Self-starter and excellent problem solver.
- Ability to retain new information.
- Ability to safely lift 35 pounds
