The Case Manager position utilizes the Brain Injury First software system.
All staff members at NLES are responsible for our participants when they are under our auspices, as well as making sure the environment remains clean and safe. Professionalism is expected at all times, in appearance and behavior.
All staff should be familiar with NLES philosophy and policies. We work for the survivors of brain injury we serve; they are to be treated with courtesy and respect at all times. No Limits members are always our first priority.
All staff at NLES, unless otherwise stated in their job description, are responsible for direct participant supervision and required documentation, as well as any additional duties as directed. First Aid/CPR certification and a valid driver’s license are also required (driver’s licenses must be approved by our vehicle insurer before staff may drive agency vehicles). Staff must have the physical ability to frequently lift 20 – 50 lbs, regularly lift 10 – 20 lbs, and the ability to bend down frequently and secure wheelchairs in agency vehicles.
Responsibilities that are specific to the Case Manager position are listed below:
- Provide case management services to survivors of brain injury and input all required documentation into the BI First software
- Work closely with the Clinical Director
- Assist as needed with NLES needs
- Notify Director of any materials or repairs needed in a timely manner
- Obtain and maintain annual ACBIS (Academy of Certified Brain Injury Specialists) and/or QBISP certification within 18 months of hire at agency expense
- Obtain and maintain Person Centered Planning training
- Make regular case note entries and complete all required BI First documentation in a timely manner
- Assist in accreditation/evaluation readiness
- Liaison with other area service providers and organizations and support them in providing quality services to survivors of brain injury
- Communicate information related to Case Management to other staff as needed (i.e. outreach member information/day program member information)
- Make routine visits to and/or maintain contact with Case Management members
- Participate in community and/or other relevant provider meetings & trainings
- Participate and attend care plan/or other member meetings
- Transport members as needed
- Utilize and maintain the Brain Injury First Software System
- Continue to expand on Case Management and Outreach skills as needed
- Ensure that ISPs, including goals, are maintained and implemented for active members
- Actively participate in the Change Control Board and other committees/boards/community organizations as directed on behalf of NLES
- Complete all other duties as directed
Compensation:
- PT 20-hour/week position with some paid holidays and pro-rated vacation time (6 PTO days per year in Years 1 - 5) as per policy. Pay is $21/hour, plus $250/month benefit subsidy, no other benefits or subsidies.
