Essential Functions
- Provides nursing and case management services within a mobile intensive community treatment program.
- The Program for Assertive Community Treatment (ACT) provides intensive behavioral health interventions to adults who have frequent and/or lengthy history of psychiatric hospitalizations due to difficulty controlling symptoms of Schizophrenia, Schizoaffective Disorder and Bi-Polar Disorder, and who also have a history of severe difficulty tending to their basic needs while living in the community.
- Works closely with other staff within PACT using a team approach.
- Provides knowledge and support to the PACT team regarding medication issues, such as side effects and correct management of medications, medical conditions and will provide brief medical assessments as needed to determine further care.
- Requires extensive coordination of care with outside providers for PACT consumers. Primary supervision for nursing issues will be provided by a Lead RN, and administrative and clinical case management supervision will be provided by the PACT Supervisor.
Minimum Qualifications
- Associates, Bachelor, or Master's Degree in human services field and one year direct clinical experience with mentally ill population.
- RN or LPN license issued by the VIrginia Board of Nursing.
- Must be QPPMH, QMHP or QMHP-T eligible.
- Must have the ability to perform blood draws and injections in the office.
- Must also have a valid Virginia driver's license with a positive point balance.
