Full Description

VitalCheck Wellness, on behalf of Photon Medical Group, is seeking a culturally sensitive and compassionate Case Manager (LPN) to join our team. Patient care will be provided in-person and virtually at one of NYC Health + Hospitals' Humanitarian Emergency Response and Relief Centers (HERRCs) making a difference in the lives of migrants.
Responsibilities

• Reviewing all incidents and guest notes for any assigned guests every shift.
• Prioritizing guest outreach for assigned guests by incident or by other intervention needs communicated by another Case/Social Worker (e.g., previous shift).
• Completing guest outreach for all priority cases every shift.
• Scheduling any incomplete outreach for next shift(s).
• Completing rounds on all guests according to case load.
• Guest episode and crisis response.
• Addressing and managing issues raised by guests or HERRC site staff.
• Maintaining guest confidentiality.
• Providing input into policies and processes to Social Worker or Vendor leadership.
• Following escalation protocols as outlined.
• Planning and communicating to HERRC leadership specific accommodations for guests with specific needs following events or incidents (e.g., mobility, meal accommodations).

Requirements

• Graduated from an accredited Nursing school; or Master's Degree in Social Work, migration studies, human rights, counseling, or related field; or equivalent experience.
• 2-3 years of experience in community-based services, social services, immigrant advocacy, community organizing, and/or humanitarian relief, supporting and advocating for immigrants and asylum seekers.
• Experience training, supervising, and mentoring staff in a case management setting.
• Experience in a healthcare or hospital setting is highly preferred.
• Knowledge of New York State regulations and guidelines for social work practice.
  
• Experience in community-based services, shelters, immigrants; volunteer or mission experience with disaster relief, refugee, humanitarian relief, preferred.
• Empathy and sensitivity to the needs of diverse patient populations.
• Bilingual in Spanish a Plus.
• Strong interpersonal and communication skills.
• Ability to work effectively and independently in a fast-paced and dynamic environment.
• Proficiency with computers and electronic medical records systems.
• Must be able to commute to New York City and Boroughs.
