Pibly Residential Programs has a vaccine policy for employees.
1. Coordinate and manage a caseload of approximately 18-20 class members who are adults with mental illness living in community care housing.
2. Provide at least two monthly face-to-face visits in each class member’s apartment observing and documenting changes in the mental status, use of the apartment, safety concerns and need for repairs.
3. Provide clinical services such as assessment, counseling, link-to and coordination of services and advocacy as needed.
4. Provide hands-on training and assistance of essential independent apartment living skills such as medication management, cooking, shopping, and budgeting on an as needed basis
5. Provide orientation to each class member to assist in engaging with the community
6. Assist each class member with obtaining/maintaining entitlements
7. Develop client centered service plans with class member’s input
8. Inform the Program Director of issues endangering the well-being, safety, health or mental status of all Adult Home program residents
9. Maintain ongoing contact with collaterals, treatment teams and other service providers.
10. Escort class members, on an as needed basis, to appointments in the community
11. Escort class members to emergency rooms when indicated, which may require mandatory overtime
12. Attend bi-weekly clinical meetings with the Program Director
13. Maintain accurate and current records in the AWARDS database at all times
14. Accurately calculate each class members’ portion of the rent annually and as their income changes according to OMH guidelines
15. Assist class members in communicating with landlords and management companies as advocacy training
16. Participate in pre- and post- class member transition calls, communicate and meet with (as needed) health home care managers to assist in the care and coordination of class member services
17. Assist class members in selecting furniture
18. Prepare apartment, utilities and food shopping prior to scheduled move in day
19. Assist in planning and attending social events for class members in the community
20. Update arrival and departure time in AWARDS attendance section on a daily basis.
21. Attend annual anti-sexual harassment training.
22. Provide On-Call Coverage as needed on the weekends
