General Description:
The key purpose of this position is to assist clients with maintaining housing through financial assistance. The Case Manager conducts assessments, determines eligibility, links individuals/families with community based services and resources, manages financial assistance funds and provides case management services in conjunction with community partners. The Case Manager is responsible for ensuring that clients receive sufficient and appropriate care and that the mechanisms to end homelessness are being accessed effectively. The Case Manager will also utilize the Salesforce system and work with the team to ensure program compliance.
Essential Responsibilities:
- Knows and understands all aspects of each program’s contractual requirements.
- Ensures that the program complies with all regulatory standards including local, state, and federal guidelines and meets or exceeds the MUST’s standard of excellence requirements.
- Ensures that all services are properly documented in Salesforce.
· Meets required benchmarks for collection of program income; as applicable.
- Provides case management services to those assigned on case load.
- Screens all applicants and determines eligibly of applicants; process financial payments and/or makes appropriate referrals as needed.
- Orientate clients on program policies and procedures
- Maintain client charts and records assigned to caseload and monitor staff charts to ensure records are properly maintained.
- Make appropriate referrals as needed.
- Other duties as assigned.
Essential Job Requirements:
- Bachelor’s degree in Social Work, Human Services or related field.
- 3+ years of experience working with mental health, disabled, veterans and/or diverse populations
- Proficiency in Microsoft applications including Word, Excel and Outlook, Salesforce reporting systems; Client Track experience preferred.
- Strong organizational, verbal, and written communication skills.
- Strong problem solving and conflict resolution skills.

- Ability to establish and maintain relationships with community partners.
- Excellent customer service skills.
- Personal qualities of integrity, credibility, and a commitment to and passion for MUST’s mission.
- Must have a valid Driver’s license and current insurance.
Physical and Work Requirements:
- Work is performed both in the field and in an office environment.
- May be required to lift up to 20 lbs.
- Requires sitting, standing, sufficient mobility and physical reflexes.
- Must be able to travel to different sites and locations. Travel typically two to three times per week.

Pay: $38,000.00 - $42,000.00 per year
