Case Manager I/Ii - Drop-In Center Modesto

One (1) full-time (40 hours/week) Case Manager opportunity located at our Drop-In Family Resource Center in Modesto, CA. The Family Resource Center’s mission is to provide and coordinate services through creative partnerships that enhance health, education and well-being for all in Stanislaus County. Case Managers are members of an integrated, multi-disciplinary service team. Case Managers provide differential response for families with children ages 0-5 who are at risk for child abuse and neglect and after care services to children 0 through 17 and their families.  Services are provided in Family Resource Centers, client homes and in the community.
Qualifications:

1. Bachelor of Arts or Bachelor of Science degree in Counseling, Social Work or related field preferred.  Degree may be waived with equivalent experience.
2. Knowledge of community resources preferred.
3. Bilingual in Spanish preferred.
4. Must be 21 years old or over.
5. A valid California driver’s license with less than two minor moving violations is required of staff who have driving duties to meet the requirements of their position and be approved by the agency’s insurance company.

Essential Job Functions:

9. Provide case management services to include community resources and referrals.
10. Assist clients with health insurance and CalFresh enrollment.
11. Administer depression and developmental screenings.
12. Conduct family and child literacy classes and children’s book distribution.
13. Provide community outreach and attend community events.
14. Facilitate or assist with parent and children’s group.
15. Complete strength based assessments.
16. Conduct health and safety classes.

Responsibilities:

1. Provide access, advocacy, resources, and linkage to clients.
2. Interact productively within a multi-disciplinary team and other community organizations.
3. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
4. Attend community events.
5. Report any suspected child abuse immediately to direct supervisor or utilize the chain of command if supervisor is not available. 
6. Report any suspected adult/elder abuse immediately to your direct supervisor or utilize the chain of command if supervisor is not available. 
7. Report any imminent danger to self or danger to others to direct supervisor or utilize the chain of command if supervisor is not available. 
8. Attend staff meetings, designated meetings, and trainings.
9. Cross training to support various roles within programs as needed.
10. Responsible for preventing and controlling infection.
11. Responsible for maintaining a culture of quality and safety.
12. Effectively use de-escalation skills and techniques for crisis prevention.
13. Properly administer the C-SSRS to help prevent suicide.
