What You Will Do to Change Lives
Under direct supervision, the Case Manager II serves as a primary support and Case Manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full Time; 8:00 am - 4:30 pm; Monday - Friday; After Hours Crisis Cell Phone on Rotation
Expected starting wage range is $17.30 - $20.86. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

To Make Referrals:
  • ICM: For institutional case management, referrals come directly from the Public Conservator's office and/or coordinated through San Diego's Department of Behavioral Health. This program only serves individuals that are on public conservatorship.

# of Members: 354 ICM members
Who We Serve: Adult residents of San Diego County, ages 60 and older, who are diagnosed with a serious mental illness. For ICM services, the individual must also be conserved and placed in a locked, residential facility.
Hours of Operation: Monday - Friday: by appointment. Crisis services available 24/7.
About the Program: At Telecare's AgeWise program, we are here to support clients in taking positive steps towards independent living.
