Your Role:
We seek a dedicated and compassionate Case Manager to join our team at OCD Anxiety Centers. A Case Manager is crucial in coordinating discharge referrals for clients completing their treatment at our program. As a Case manager, you will be expected to work closely with clients, their families, healthcare professionals, and community resources to ensure a smooth transition out of programming and into continued care.
This position also plays an important role in creating a collaborative relationship with other providers to create a healthy referral relationship. A successful candidate is someone who possesses a combination of clinical knowledge, interpersonal skills, and a commitment to supporting clients and their families on their journey to recovery.

Responsibilities:
- Working with clients, family, support networks, and healthcare professionals to implement care plans.
- Develop individualized discharge plans in collaboration with the treatment teams, clients, and families.
- Identify and coordinate appropriate referrals for continued care, including outpatient therapy, medication management, support groups, and other community resources.
- Communicate effectively with external providers to ensure continuity of care and seamless transitions for clients.
- Listen to clients’ concerns and advocate for clients’ needs throughout the discharge process.
- Stay well informed and connected with current best practices and updated community resources to provide discharging clients with the best possible care.
- Other responsibilities as delegated.
Qualifications:
- Bachelor’s degree in social work, psychology, or related field
- Previous experience in case management, discharge planning, or related roles within a mental health or healthcare setting preferred
- Strong communication and interpersonal skills, with the ability to engage and connect with clients, families, and external providers.
- Ability to work collaboratively as a part of a multidisciplinary team.
- Excellent organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
- Familiarity with electronic medical record (EMR) systems and documentation.
- Compassionate, client-centered approach to care with a commitment to advocating for individual needs
