Job Summary
The Case Managers is a resource to unlicensed care coordinators, remotely supporting field nurses with complex and high risk patients needing care at home. The RN/LVNCase Manager is also responsible for implementing, identifying and coordinating options and services to meet at-risk patients' healthcare needs. The Case Manager plans and oversees the patients' visits and schedules, according to authorizations, patient care needs, and clinician availability. The Case Manager must maintain excellent relationships with the field clinicians and patients, be aware of clinician's special skills and analyze each visit type while determining patients' care needs and the clinician suitability. The Case Manager reports to the Director of Nursing (DON).
Additional Responsibilities
- Accept new referrals on daily basis and create schedules based on the referral requirements.
- Organize active patient schedules based on visit type requirements.
- Manage field staff availability schedule and communicate with other care coordinators.
- Provide accurate daily schedule and routes to field nurses.
- Performs other duties as assigned related to patient care management.
