Form of workConcourse House, HDFC is a not for profit providing services to families in our Northwest Bronx community. We provide those who live here with services they need to strive as families. Those services include transitional housing, case management and child care, through our shelter, social work field staff and day care programs respectively.
It is our hope that every family that we work with is able to live a healthy independent life by utilizing the resources we provide. We are seeking case -managers to work in the field with formerly homeless families who are now living independently,but will benefit from support services. The Case Manager will provide on-going social services to the client and family as needed.
The responsibilities include: Assesses client's needs and develops initial and on-going clinical plan for care, follow up and update regularly Assist with yearly apartment recertifications Updates plan at specific intervals, and as needs change in client's condition or circumstance Performs and manages effective case management Maintains detailed, accurate and timely progress notes and other documentation Develops inventory of needs and devises a plan to meet those needs Provides linkage to resources and make referrals to community agencies Works with team members and provide outreach for and engage resistant/hard to reach clients Advocates and provides assistance with all social, educational and health needs Assist with maintaining a healthy household by developing a plan to keep the home in good standing both physically and financially Knowledge of current Housing Programs and vouchers, assist with arrears and relocation when neccessary Attends appointments with client as needed Outreach and Training as well as Network and liaise with outside agencies and make referrals Attends staff meetings, trainings and supervisory sessions as required * Bachelor's degree in Human Services or related field preferred. MSW a plus. Salary starting from $53,000 up to $55,000 with experience, plus benefits.
