CSC'SCase Manager - Behavioral Health’s Purpose:
As a Behavioral HealthCase Manager at CSC, You will:
- Work directly with patients and clinic providers.
- Conduct pre-screenings and assessments.
- Develop comprehensive care plans.
- Coordinate and monitor medical treatments.
- Connect patients with community resources.
- Provide ongoing follow-up and support.
- Participate in team meetings and contribute to clinic development.
Minimum Qualifications:
- High school diploma required; Bachelor's degree preferred.
- 1 year of experience in Behavioral Health, preferably with case management.
- Knowledge of Asian immigrant and refugee communities.
- Bilingual/bicultural English and Chinese required.
- Excellent interpersonal, communication, and organizational skills.
- Commitment to cultural competency and client-centered care.
- Successfully meet the post-offer/pre-hire physical and receive an “Eligible for Employment” status for Level II Background Check required by the State of California.
