Position Summary
The Case manager is responsible for individualized management of hospital and healthcare services necessary and appropriate to the provision of high-quality, cost-effective patient care throughout the continuum of care. He or she works with acutely ill and/or high-risk adult Behavioral Health clients suffering from substance use disorders. The Case Manager develops collaborative, outcome-oriented plans of care for each patient, acts as a patient advocate and provides mechanisms and support to empower patients to make responsible healthcare decisions, while guiding appropriate utilization of resources and services to achieve expected outcomes.
Responsibilities
- Candidate must work independently.
- Performs needs assessment, treatment plan development and implementation.
- Mandatory reporter for all signs of abuse.
- When appropriate, complete Duty to Warn.
- Performs patient assessments to identify pertinent problems/needs.
- Links patient with resources for treatment needs.
- Organize assessment data into an individual plan of care with anticipated outcomes.
- Provide patient education regarding diagnosis and treatment options.
- Manage avoidable ED visits and hospital admissions.
- In home visits for Behavioral Health patients requiring support, direction, and encouragement.
- Local transportation of patients for appointments and treatment services.
- Maintain CPR certification. (Health System provided).
- Maintain Aegis training. (Health System provided).
Requirements
- Candidate must possess CDCA, SWA, or LSW and have associated licensure.
- Knowledge of evidence-based practice and substance use disorder management protocols.
- Excellent verbal and written communication skills.
- Excellent computer skills. Knowledge of Epic EMR a plus.
- Behavioral Health Case Management experience preferred but not required.
- Must have CPR certificate. (Health System will provide).
- Must pass background check.
- Must have a valid driver's license and current proof of insurance.
