General Duties: Case Managers are a key component of the Summit County Juvenile Court Multidisciplinary Representation Team. They are assigned to assist with the civil legal representation of parents, guardians and/or legal custodians of children involved with abuse, dependency and/or neglect cases (hereinafter referred to as “client”) at the request of the attorney representing the client. The Case Manager is an agent of the attorney and becomes a member of the parent’s legal team and is subject to the same bounds of confidentiality and attorney-client privilege.
Qualifications: Bachelor’s or higher-level degree in social work, human services or a closely related field; or a Bachelor’s degree in another field with a minimum of one year of work experience in a social work or human services position; Preference will be given to individuals who are Licensed Social Workers in the State of Ohio.
TASKSPERFORMED:
Case Manager:
- Practice client engagement strategies to build trust and relationships with clients
- Provide direct service focused work in the client’s community
- Support clients in identifying barriers
- Provide insight to the legal team from a social work perspective
- Gather information to strengthen case advocacy and court preparation
- Conduct such other duties as may be assigned as a member of the client’s legal team, consistent with their training and qualifications
Role:
- Encourage client to work case plan
- Work with client to problem solve barriers and diffuse anger
- Provide system navigation support
- Provide ongoing, in-person visits with clients to assess progress and identify self-directed goals.
- Attend Court hearings and meetings at Summit County Children Services as client support
Assist client in Obtaining:
- Services to address issues of substance abuse, mental health, domestic violence
- Assist with issues of housing, employment, budgeting, basic needs
- Assist with child(ren)’ s appointments and meeting (school, medical, counseling)
- Develop sustainable community support
Gather Information:
- Observe visits and provide parent coaching
- Assist parent in identifying relatives and/or other family supports to placement and visit supervision
- Connect with service providers regarding progress in services on behalf of legal team
Advocate:
- Attend SCCS and other meetings to assist parent
- Assure that appropriate referrals are made to allow parent to complete case plan
- Make individualized recommendations for services
- Maintain contact with SCCS and other professionals and clarify expectations as necessary.
Conditions under which work is performed: Performs duties under the direction of the Grants Administrator; works regular Court hours typically, but may be required to work outside of the normal Court hours when necessary to meet client needs; may be required to travel outside of the state for conferences or training at times. May be required to operate a County vehicle.
