Position Summary
The AELP provides classes in English for Speakers of Other Languages (ESOL), Adult Basic Education (ABE) and
HSE preparation for adults ages 18 and above. The AELP incorporates job readiness, civics, and job placement into its curriculum. The goal of the program is to help adults improve their English language proficiency, enhance literacy skills, obtain HSEs, and obtain or improve employment.
The Case Manager will complete a comprehensive assessment and a corresponding service plan with each client who demonstrates or expressed social service needs. This work will be done in collaboration with each participant to elevate their experiences, their strengths, their interests, and their voice. While in the program, each participant will meet with the Case Manager regularly to work on barriers to program persistence and success, including the provision and follow through with referrals to other service providers. The Case Manager will also complete all necessary data entry related to this work and support the larger team with other program activities, such as outreach and recruitment, workshops, post-exit placements, etc.
Essential Job Functions and Responsibilities
- Assess each applicant for the program fit and match with contractual enrollment criteria.
- Complete a thorough assessment and design a corresponding service plan in collaboration with each qualifying applicant and complete necessary enrollment paperwork.
- Meet with each student to identify interests and strengths and develop an individual education plan. The plan will clearly list long-term and short-term educational goals with specific action steps required to meet each goal.
- The counselor should help participants to develop skills to improve study habits, social skills, and time management.
- Assure quality, timely data entry into and documenting each interaction with participants in DYCD’s Participant Tracking System and agency’s Salesforce instance.
- Support a culture of professionalism designed to teach and cultivate real-world working experience for the youth and young adults served.
- Assist in identifying and developing services and programs not currently being offered.
- Collaborate with balance of program team to fulfill all program functions and services that may need additional support.
Knowledge, Skills and Abilities
- Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
- A passionate commitment to OBT’s mission and vision.
- A track record in providing comprehensive case management services to immigrants. Familiarity with the public and nonprofit social service systems and entities that target this population.
- Demonstrated experience developing programmatic systems to enhance program outcomes.
- The ability to establish strong working relationships with team members, colleagues, participants, and external stakeholders.
- The ability to consistently develop and refine the program model and processes to create successful outcomes for participants and staff.
- Experience working on multiple projects and balancing competing priorities.
- Strong skill set in data management and data tracking; demonstrated experience managing data collection, analysis, and reporting.
Education and Experience/Training
- Associate degree in Human Services (bachelor’s degree preferred)
- A minimum of 2 years’ experience in a Case Manager or comparable role.
- Bilingual – Spanish or French
