```Job Summary```
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and advocating for the care of patients in various healthcare settings. ```Duties```
- Conduct comprehensive assessments of patients' needs and develop individualized care plans

```Experience```
- Previous experience as a Case Manager or similar role is preferred
- Knowledge of medical terminology and case management principles
- Familiarity with behavioral health, managed care, pediatrics, acute care, and primary care settings
- Strong communication skills, both written and verbal
- Ability to work collaboratively in a multidisciplinary team
- Proficient in using electronic medical records systems
