Dream Investment Strategies, Inc. (“DIS”) is a social services agency that provides supportive employment and support housing services to at-risk members of the community.
DIS is seeking a dedicated and compassionate remote Case Manager (flexible schedule) to join our team. In this role, you will be responsible for assisting individuals with disabilities in securing and maintaining meaningful employment opportunities and achieving housing stabilization.
The position entails meeting with clients, creating action/goal plans, utilizing available resources to help clients achieve their goals, conducting regular weekly check-ins with clients, managing client case files, and meeting with team members to provide updates.
Core Position Duties:
Client Assessment and Goal Setting: Conduct comprehensive assessments to understand the strengths, abilities, and goals of individuals with disabilities, collaborate with clients to develop personalized employment plans, setting achievable goals for skill development and job placement, and collaborate with clients to develop personalized housing plans, setting achievable goals for securing and maintaining stable housing.
Resource Coordination: Establish and maintain relationships with local businesses to identify suitable employment opportunities, actively promote clients to potential employers and facilitate the job placement process, provide guidance on resume development, interview skills, and other job-seeking strategies, identify and connect clients with available housing resources, rental assistance programs, and supportive services, and establish and maintain relationships with landlords, property managers, and housing agencies to facilitate housing placements.
Ongoing Support: Offer continuous support to clients in their new work environments, addressing any challenges or concerns that may arise, work closely with employers to ensure a positive and inclusive workplace for individuals with disabilities, and implement strategies to enhance job retention and career advancement.
Advocacy and Education: Advocate for the rights and needs of individuals with disabilities in the workplace, and educate employers, coworkers, and the community on the benefits of inclusive employment practices.
Documentation and Reporting: Maintain accurate and up-to-date case records, documenting client progress, achievements, and challenges.
Networking and Collaboration: Collaborate with community resources, vocational rehabilitation agencies, and other service providers to enhance support for clients, attend relevant meetings, workshops, and conferences to stay informed about industry trends and best practices, and collaborate with community partners, housing authorities, and service providers to expand housing options and resources, represent the organization in community forums and advocacy initiatives related to affordable housing.
Qualifications:
●Self-starter attitude is absolutely necessary.
● Previous experience in employment services, case management, or a related field preferred.
● Strong communication and interpersonal skills.
● Ability to work independently and as part of a team.
● Proficient in Google Drive/Google applications and case management software.
We would prefer a candidate with a social services background, but are open to candidates who display competence and a desire to learn the position.

This position is flexible and DIS will work with the candidate to create a work schedule that suits both client and employee needs. You will need to be available to meet with clients during business hours, but all other work may be completed at your discretion.
Pay: $23.50/hr, 1099 position. You are free to choose your hours and the number of clients you are assigned will reflect the hours you are available to work. A PT Case Manager available to work 20 hours a week will be assigned no more than 15 clients at any given time.
Growth Potential: This part-time position has the potential to grow into a full-time position by mid-June 2024. Should you desire to transition into full-time W-2 employment, you will have the option to do so.
Hiring Timeline: We are looking conduct interviews the first week of April and will be seeking to onboard our new employee by April 15, 2024.
