Job Description
The Case Manager facilitates patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress. They oversee and coordinate with the medical team to ensure patients receive personalized and quality care.
Location: Palm River & Plant City
Duties and Responsibilities
- Promotes and reinforces patient centered medical home concepts with patients and staff.
- Analyzes the physical and mental health needs of a patient to recommend appropriate care and treatment plans.
- Serves as a point of communication between medical workers, administrative staff, patients, and family involved in the treatment process.
- Ensures patient records are thoroughly and accurately updated.
- Arranges referrals and/or recommends patients to specialists when needed.
- Ensures that appointments are made and kept by both staff and patients.
- Evaluates the progress and recovery of a patient with the goal of discharging them from medical care.
- Presents QI findings at committees.
- Reviews current literature and attends training sessions and seminars to keep informed of new developments in the field.
- Understands PCMH guidelines and incorporates them into daily practice.
- Performs other duties as assigned.
Qualifications / Education / Licenses
- Bachelor’s degree in a related field required.
- One year of experience in a clinical setting and patient access.
- Three to five years of experience preferred.
- Registered Nurse license in the state of practice.
- CPR certification required.
- First-aid certification preferred.
- Bilingual in English and Spanish preferred; must be able to read, write and speak English.
- Basic computer knowledge; Microsoft Office experience, sending e-faxes and email, document with electronic health records and/or authorization system.
- Annual completion of OSHA Training.
