Job description

Job Summary:

Hands Of Healing’s Case Manager will ensure the safe and timely release of UC within ORR timeframes. Within twenty-four (24) hours of placement, the CM will conduct and provide UC with a program orientation. The CM will work collaboratively with the designated Clinician to address the immediate needs of the UC through the development of the UC Assessments. The CM will work with the designated Clinician to develop safety plans for UC when: a history of abuse is identified; there is specific need identified (medical, physical, or behavioral), or when situations involving non-routine events take place that involve UC in care. The CMs plan for the safe and timely release of UC by gathering required information, verifying family relationships, requesting supporting documentation, and screening for risk and exploitation. Within twenty-four (24) hours of the identified sponsor, the CM will send the sponsor the FRP (family reunification packet).

They are also responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file as per HOH’s policies and protocols. The Case Managers are required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours, weekends, and be on-call.

Essential Functions:

  • Maintains communication with the designated Clinician to ensure all UC identified to be at risk or a victim of sexual abuse or sexual harassment, are appropriately referred and provided with in care and aftercare services. Based on information obtained, a release recommendation is identified.
  • Maintains contact with Case Coordinators regarding status of release recommendations, and to identify transfer recommendations for UC who do not meet placement criteria.
  • Document the Safety and Well Being Follow-Up Call and report any safety concerns in accordance with ORR Safety and Well Being Follow-Up Call.
  • Ensure the placement in care is appropriate and services provided meet the needs of the UC, CMs will have weekly interaction with UCs.
  • Ensure ORR safe and timely release policy revisions are appropriately implemented within required timeframes.
  • Tracks and monitors each UC’s progress to ensure the full range of program services are provided in an integrated and comprehensive manner.
  • Collaborate with the Reunification Specialist to provide guidance and maintain current on ORR policy and release requirements to ensure timelines are met
  • Maintain ongoing communications with the PSA Compliance Manager to ensure all relevant in care and after care services are provided to UC, in accordance with ORR PSA Policy.
  • Serve as primary liaison between ORR, DHS, Border Patrol, legal services and Immigration Court.
  • Plan and coordinate internal departmental meetings, case staffing, repatriations, attorney-UC visits, LOPC appointments and referrals to the Young Center for Immigrant Children’s Rights.
  • Manage the safe and timely release process, conduct monthly random file audits, maintain knowledge of all serious incidents (SIRs), review safety plans, assist in the coordination of the HOH Evacuation Process.
  • Communicate all placement referrals with Lead Clinician and remain on call.
  • Submit daily reports to ORR including the daily capacity count (as maintained by YCWDECM), daily medical/special needs report, reunification and discharge reports and safe and timely release case status.
  • Ensure that the continuation of services, case management and reunification services will be provided 7 days a week.
  • Attend case staffing with funding source representative.
  • Attend all organizational required trainings.
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practice are adhered to.
  • Attend training that will enhance professional growth as Case Manager or other topics as deemed appropriate.
  • Must assist in the evacuation of youth as needed due to inclement weather conditions, natural disasters, or other unforeseen occurrences.
  • Travel as needed for trainings, conferences or to transport youth to destinations located within the U.S.
  • Maintain a safe, clean, and hazard-free work area.
  • Ensure the proper supervision of youth at all times.
  • Able to react to change productively and handle other essential tasks as assigned.

Qualifications:

  • At minimum, the Case Manager must hold a Master’s degree Social Work, Psychology, Human Services, Counseling, or other social service field; or a bachelor’s degree and at least 1 year of progressive employment experience that demonstrates supervisory and case management experience.
  • Must be computer literate with working knowledge of Microsoft Windows (Word Perfect, Microsoft Word, Excel)
  • Must possess a valid state driver’s license and be eligible to drive to facilitate program services as required by Cooperative agreement.
  • Required to work a flexible schedule to facilitate program services.
  • Cleared Tuberculosis test results.
  • Cleared background check from appropriate entity.
  • Must be Bilingual (English/Spanish)
  • Cleared pre-employment and random drug test results
  • Must be at least 21 years of age at the time of hire.

Physical Demands:

Must be able to obtain and perform Emergency Behavior Intervention Training (EBI)/Crisis Prevention Institute (CPI) certification and First Aid Certification (CPR). Must be able to supervise clients indoors and outdoors as necessary. Bending, stooping, and lifting up to 20 lbs. required to complete daily tasks. Job requires extended periods of sitting and use of various office equipment.

Work Environment:

Work is at a youth care facility at which minors receive multiple services. Two shifts per twenty-four-hour period. Work shifts are subject to change. Noise levels vary from mild to moderate. Predominantly indoors, occasional outdoor monitoring or activities.
