Job Title:Case Manager – Veterans’ Housing Stability Program (VHSP)
Location: SAMMinistries, 1919 NW Loop 410
Reports To: Manager of Prevention Services
FLSA Status: Exempt

SUMMARY
The VHSP Case Manager is an experienced social services worker who will provide comprehensive case management services and housing support to veterans and dependents, or surviving spouses and dependents, so they can remain in their home or re-gain housing after an episode of homelessness. Actively works with families and individuals in developing a service plan consisting of goals, objectives, and tasks that will provide the client an opportunity for Housing Stability. Facilitates access to community resources to help the family increase self-sufficiency. Conducts outreach efforts with social service agencies working with veterans and/or their families who are at imminent risk of losing their housing or who are currently experiencing homelessness.
ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Conduct client phone screenings and eligibility assessments for potential candidates.
  • Complete all necessary documentation in accordance with the funding source to verify financial hardship, process financial assistance for the household and compile and maintain client case file.
  • Provide rapid re-housing services for qualifying households who meet HUD’s definition of literal homelessness.
  • Use evidence-based practices in service delivery such as Client Centered Care, Motivational Interviewing, Harm Reduction, Trauma Informed-Care and Housing First practices.
  • Provide field-based case management services in the areas of, but not limited to independent living skills, money management, community integration, linkage to mainstream benefits, employment services, primary and mental health care, substance abuse and any other services needed to support clients in reaching their identified goals while promoting family self-sufficiency and housing retention.
  • Work in collaboration with property management to help resolve any concerns or issues that may arise with the client to help maintain Housing Stability.
  • Assist the client in working toward strengthening sustainability in permanent housing.
  • Maintain documentation in the Homeless Management Information System (HMIS) and adhere to data quality standards as well as compliance with contractual obligations and SAMMinistries’ policies.
  • Conduct required follow-up assessments/surveys with individuals and families who have received assistance.
  • Meet regularly with the Manager of Prevention Services to review program key performance indicators (KPI’s), program progress and to staff clients.
  • In town travel will be necessary to facilitate in-home case management services.
  • Attend veteran service provider bi-weekly meetings.
  • Attend all mandatory departmental meetings and/or trainings.

KNOWLEDGE, SKILLS, AND ABILITIES
  • Two years of previous case management experience is required.
  • Knowledge of local housing continuum and program requirements preferred.
  • Knowledge of fair housing and tenant rights preferred.
  • Excellent time management skills and the ability to effectively communicate both orally and in writing are essential.
  • Experience and knowledge in providing services to disadvantaged populations and effectively collaborating with partner agencies is essential.
  • Knowledge of basic social work principles and the ability to demonstrate sensitivity, maintain confidentiality, and to work with clients in an equitable, but flexible, manner are essential qualifications.
  • Must provide services in a culturally sensitive and professional manner
  • Carry out operations in accordance with the organization's policies and applicable laws.
EDUCATION and/or EXPERIENCE
  • Bachelor's degree in social work or related field.
  • Experience working with families and individuals in crisis.
  • Experience working with at-risk populations, including families and individuals who may have experienced homelessness, may have behavioral and/or physical health challenges, and have extremely limited income is necessary.
  • Veteran of the US Armed Forces is preferred
LANGUAGE SKILLS
Bilingual (English/Spanish) is preferred.
MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as addition, subtraction, division and percentages.
REASONING ABILITY
Must be sensitive to the dynamics associated with homeless families and individuals. Must be able to react swiftly and effectively to crisis situations. Must possess good conflict resolution skills. Logic and ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
CERTIFICATES, LICENSES, REGISTRATIONS
A valid Texas Driver’s License, a reliable motor vehicle and liability insurance are required.
WORK ENVIRONMENT
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic health and safety practices. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure. May be required to work flexible hours to include evenings and weekends.

The employee may be required on occasion to lift and or carry up to 20 lbs.
