Essential Functions:

• Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day
• Communicate necessary information to others as appropriate
• Oversee Care Management team and activities.
• Act as a Case Management liaison between physicians/community partners/other departments within FHW/patients and their families and FHW with the goal of improving satisfaction by enhancing the patient experience.
• Review, analyze and communicate opportunities between staff, departments, physicians, office staff and patients.
• Organize and coordinate admission and discharge processes to ensure consistency and excellent customer service.
• Supervise Utilization Review Coordinator/Insurance Specialist and Case Managers and direct and review work activities of staff as necessary.
• Communicate effectively and professionally with all service areas, including finance, referring facilities and physician practices
• Develop related policies as directed and in collaboration with team members and other peers.
• Ensure that Case Management Team collaborates with other FHW staff to promote offered services and improve communication and services.
• Communicate effectively and professionally with all service areas, referring facilities and physician's practices.
• Work closely with other departments to effect a great patient experience consistently.
• Participate in Community Meetings with other providers as needed
• Attend monthly house leadership meeting on the first Monday of the month.
• Coordinate team meetings per team need. Executive involvement in one/month or more frequently as need is identified.
• Manage time sheets of Case Management staff to include PTO requests, holidays and on call time.
• Assist with performance evaluations as directed.

Education:
Education
Minimum Associates degree or higher in a medical related field
Licenses
No professional license required.
Certifications
No professional certification required.
Experience
1-2 years in an acute care setting
Wage Range : $23.00 - $31.61
Immunizations required for employment
