Job description

Sibley Memorial Hospital is a non-profit, full service 318 bed acute care community hospital serving the Washington, DC area. Sibley offers medical, surgical, intensive care, obstetric, oncology, orthopedic and skilled nursing inpatient services, as well as a 24-hour Emergency Department. Sibley's campus is also home to Grand Oaks, an assisted living residence; a medical building with physician offices and ambulatory surgery and imaging centers; and the new patient tower with 200 spacious, private rooms.
The Case Management Assistant (CMA) plays a crucial role in supporting licensed social workers or nurse case managers in transition planning activities, and facilitating appropriate placements in rehabilitation facilities or alternate levels of care. They collaborate closely with patients, families, and vendors, coordinating services such as post-acute placements, transportation arrangements, and scheduling post-discharge appointments. Additionally, the CMA ensures accurate documentation in electronic medical records, resolves any barriers or issues related to facility choice, and provides essential information in discharge packets to accompany patients to their designated facilities.
Who Should Apply:

  • High School Diploma required; an Associates Degree in Health Science or related field preferred.
  • Ability to exchange factual information, schedule appointments, greet visitors, explain medical center policies, and relay messages with courtesy and tact. Sensitive situations may be referred to appropriate personnel.
  • Analytical skills to resolve problems using basic scientific, mathematical, or technical principles.
  • Minimum one year of previous hospital or long-term care experience in Case Management and admissions.
