Job Summary and Qualifications
The Case Management Assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions
- You will coordinate with RN case manager and social worker for support needed. Communicate updates on progress throughout the day. Refer patient questions back to the case manager/social worker.
- You will provide clerical support in multidisciplinary rounds, including updating NATE Tempo at direction of team.
- You will use web-based referral system to manage post-acute care (PAC) referrals. Includes sending appropriate clinical information from the medical record required for the level of care. The case manager or social worker will make the initial referral.
- You will monitor patients with PAC referrals and intervenes to expedite discharge.
- You will prioritize workload to ensure a timely contact and intervention with PAC providers.
- You will send additional information to PAC provider when requested.
- You will communicate with RN case managers/social workers regarding the status of PAC referrals, bed availability, and other barriers that cannot be resolved.
- You will communicate results of PAC referrals with the patient, including having the Patient Choice letter signed.
- You will monitor Medicare patient’s planned discharge dates and delivers the Important Medicare Message (IMM) to the patient per facility policy.
- You will based on facility practice; set up transportation/send information to Transfer Center and generate discharge packets for PAC transfers.
- You will utilize Case Management systems, standards, and processes.
- You will maintain knowledge of and actively develops relationships with PAC providers.
- You will demonstrate knowledge of regulatory requirements, HCA Ethics and Compliance policies.
What qualifications you will need:
- High School Graduate / GED
- Associate Degree preferred
- Medical knowledge and terminology required
- Minimum of one-year experience in a health care setting required
- Or equivalent combination of education and/or experience
- Healthcare professional (CNA, EMT) preferred
