POSITION SUMMARY:
The Case Management Administrative Assistant assists the Case Managers with various daily healthcare service operations. The Case Management Administrative Assistant will be vital in maintaining documents and distributing documents to responsible stakeholders. JOB SPECIFIC RESPONSIBILITIES:
1. Organize and maintain Admission and Discharge Case Management folders.
2. Build initial Case Management file on scheduled admissions: face sheet, insurance info, medical records, blank release of information form, patient labels, etc.
3. Fax reports and clinical information to stakeholders.
4. Thin and expunge old Case Management files.
5. Manage schedule for MTP planning schedule/changes & distribution.
6. Assist Case Managers with preparations for treatment planning conferences. (MTP)
7. Schedule Family and Team meetings with all required stakeholders.
8. Schedule patient appointments, schedule transportation, communicate with all stakeholders, and manage transportation logs.
9. Coordinate the school registration process in collaboration with Case Management, Patient Advocate, School District, and Caregiver.
10. Assist with discharge planning for resources and placements, check on placements accepting of payors, bed availability, services provided, cost, etc.
11. Fax prescriptions to pharmacies and follow up as needed to ensure medications are filled before discharge.
12. Fax orders to (Durable Medical Equipment) DME and suppliers and coordinate delivery schedule as needed.
13. Complete discharge satisfaction surveys.
14. Performs other duties as assigned.
POSITION QUALIFICATIONS:
EDUCATION:
- High school diploma or equivalent required.
EXPERIENCE:
- Two (2) years of hospital experience or healthcare experience is preferred.
