Provides personal care, comfort measures, and supportive care to clients in their homes as assigned by RN or supervising PT, OT, or SLP (when nursing services are not being provided). Assists with prescribed exercises as instructed by therapists. Assists patients with activities of daily living and with taking self-administered medications. Observes patients’ condition and reports changes to the Case Manager promptly. Performs homemaking services in the home, as assigned, when a patient or family cannot meet these responsibilities. The Home Health Aide works under the direction and supervision of a registered nurse or therapist, in accordance with an established plan of care, Company policies and applicable state, federal, Medicare, Medicaid, and HIPAA regulations and standards and third party payer guidelines.
The home care aide provides non-medical personal care and supportive services that are intended to assist clients with activities of daily living and supportive living services including, but not limited to personal care, medication reminding, housekeeping services, personal laundry, cooking, shopping, escort to and from appointments, maintenance of household records, and companionship. The Home Care Aide works under the direction of the Supervisor and cooperates with the Scheduler to provide services in accordance with the home services plan of care and Agency policies and applicable regulations and standards.
While performing the essential tasks of the job, the employee drives to patient homes and provides caregiving in patient homes. Employee is exposed to common risks associated with walking and traveling in a variety of communities and neighborhoods. Work area mobility requirements include steps/stairs, handrails, protrusions, narrow access and cluttered space, walking on varied surfaces. The break cycle varies at the employee’s discretion when planning the day’s activities. On average, 80% of the job location is inside and 20% outside, in car and walking into residences. The employee is exposed to seasonal outdoor temperatures. The employee is exposed to medical waste, fumes, odors and dust.

- High school or equivalent (Preferred)
