Care Ring - Lead Family Health Navigator (Lfhn)

Summary:

Care Ring - a fast-growing and highly respected community organization seeks a passionate leader for the role of a Lead Family Health Navigator for the A Guided Journey Program (AGJ). Under the supervision of the Director of Maternal Child Health and the Program Manager, the Lead Family Health Navigator (LFHN), will be responsible for ensuring the smooth operations of the AGJ program and new hire training. In-addition, the LFHN provides comprehensive services to pregnant and parenting families residing in Mecklenburg County that qualifies for the program. The LFHN also serves as a role model to the team by promoting a positive and professional demeanor and attitude towards clients, staff members, and volunteers. Working within Mecklenburg County and Care Ring's A Guided Journey structure, the LFHN ensures implementation of AGJ in accordance with established guidelines and contract requirements.

Responsibilities:

Basic Job Function

• Assist the program manager in developing strategies and providing educational opportunities to enhance knowledge and skill gaps of Family Health Navigators and support staff
• Attends relevant community events to raise awareness that promotes A Guided Journey - program
• Engage and oversees regular outreach and recruitment activities to support program sustainability
• Adheres to Care Ring 's policies and procedures and support the program vision and mission
• Recruit, trains and coaches FHN's for the betterment of the program.
• Manages the orientation and onboarding of new Family Health Navigators under the direction of the program manager.
• Develops and implements processes, procedures and policies that help FHN's manage caseloads, including proper documentation.
• Understands, supports, and coaches FHN's in AGJ and Care Ring's vision, mission, and model
• Participates in and presents at partners, community, and essential stakeholder meetings as needed to promote AGJ and Care Ring.
• Provide learning opportunities around pregnancy, post-partum, newborn and toddler education
• Adapts to and work effectively with a variety of situations, individuals, groups and systems.
• Provide comprehensive services to 25 families, or more if needed
• Maintains confidentiality and adheres to HIPAA regulations.
• Completes all participants charting in a timely manner, with accuracy and detail and ensures that the record accurately reflects the services provided to enrolled participants
• Provides information referrals to participants and community residents
• Communicates with the program manager when deadlines are not being met
• Assist the program manager in reviewing agency policies and procedures to guide implementation
• Plan and help facilitate team meetings, team communications, team schedules, and other performance management needs
• Provide program manager with feedback regarding the FHN's' daily responsibilities and volunteers' progress during training period
• Assist in evaluating each FHN through documented feedback

Qualifications and Education Requirements

• Compassionate and consistent in working with staff and clients

• Reside in Mecklenburg County
• Detail oriented
• Must be able to work a flexible schedule including some weekend and evenings
• Minimum of 3 years' experience coordinating and leading small projects
• Minimum of 2 years' experience working with maternal child health, community outreach, health education or in a role representing or supporting the underserved population.
• High School Diploma and related work experience and training preferred
• NC State Community Health Worker (CHW) certification preferred
• Excellent computer skills necessary including all MS Office Suite applications or equivalent programs and working knowledge of NC Care 360.
• Ability to document accurately, concisely and in a timely manner.
• Strong interpersonal skills required to communicate effectively in both written and oral forms, including presentations
• Valid driver's license and car insurance required.
• Current CPR required
• Proof of Covid-19 vaccination required
