SUMMARY
The Care Resource Coordinator will provide linkage to care case management using strength based approach with people living with HIV/AIDS. Assists in the development and implementation of a recruitment plan for connecting HIV infected individuals recently diagnosed or lost to care at IDP. The individual also coordinates with case management partners/health care team assuring smooth, timely transition between sites of care. Provide ongoing client referrals to appropriate community resources and services based on client need and eligibility.
QUALIFICATIONS
- 2-3 years of eperience in lieu Bachelor's degree in related field
- Knowledge of HIV/AIDS or service coordination experience with the target population
- Negotiation and conflict resolution skills to effectively serve a diverse population
- Possess problem solving, critical thinking skills and basic counseling skills
- Communicates effectively both verbally and in writing
