Description

We provide person-centered care and strive to deliver outstanding customer service to our residents, families, and vendors.
We are looking for people who can live into these values in the role of Care Partner (CP), making a difference in the lives of vulnerable adults by providing loving care to our residents as part of a dynamic team. CPs are unlicensed staff who provide direct care, pass medications, maintain electronic medical records, comply with patient safety practices, and work under the direction of the professional nurse and administrative staff. Care Partners interact with family members of residents, and work as a team with fellow Care Partners, House Managers, Site Directors, and nursing staff.
Full Time and Part Time shifts available. Shifts are AM (6:30am-3:00pm), PM (2:30pm-11:00pm), Overnight awake (10:30pm-7:00am). Shift time modifications are not available.
Wage Range: $18.00 to $23.00 per hour based on experience.
Medical/dental benefits available to FT employees. All Full Time employees and some Part Time employees accrue paid time off.
Loan forgiveness available to achieve CNA certification. Upon certification wage will increase to $19.50 per hour.
Employee Job Description:

1. Document resident needs and status in electronic medical record, to include accidents and injuries.
2. Pass medication and complete online charting (training provided).
3. Assist with resident personal cares and ADLs with attention to individualized service plans.
4. Provide exceptional customer service.
5. Maintain safe environment by complying with all safety and regulatory guidelines, policies and procedures including Minnesota's Vulnerable Adult Laws, Resident Bill of Rights, HIPAA. Use personal protective equipment.
6. Assist with outings and activities, resident guest visits, and volunteer-led activities.
7. Prepare meals and perform housekeeping services.

Requirements
EDUCATION LEVEL: High School diploma or Equivalent. Must be 18 years or older.
EXPERIENCE:

1. No prior experience or training is required. Care Partner must successfully complete training provided by Rakhma. Dementia experience preferred.
2. HHA or CNA certification is beneficial. Training in dementia care is provided.
3. CPs must successfully complete ongoing trainings, provided by Rakhma.
4. Staff must successfully complete a background check with the Department of Human Services to be eligible for employment.

To perform the job successfully, an individual must have reliable transportation and dependable communication methods to ensure punctuality and regular attendance.
Ability to stand, walk, sit, reach with hands, climb stairs, balance, stoop, and crouch. The employee must occasionally lift and/or move more than 75 pounds.
