Position: Care Navigator
Location: Nashville, TN (Onsite 5 days a week)

About this Role:
The Care Navigator is a committed, caring colleague and an important steward of Matter Health in the communities we serve. This is a great role for someone that is energetic, self-motivated, and able to work independently with high integrity. Our Care Navigators are positive intent-minded teammates determined to help support our Primary Care Providers and Patient Advocates to ensure we provide great service to our patients and Matter Members.
Matter's Care Navigator is the first face our members see when they enter a clinic and is instrumental in helping members feel welcomed. You will have a variety of clerical, clinical and membership enrollment tasks. Your daily responsibilities will include helping coordinate patients' health needs, social services and answering their non-clinical questions. It will also include partnering with the Patient Advocate(s) on events and seminars for Residents and Matter Members, engaging directly with individuals, and working to help the rest of the team coordinate and provide additional services. This role combines traditional and effective clinical operations management, problem solving, and organizational skills in a truly rewarding, multi-channel growth role.
The Day-to-Day:
You will apply your clinical and administrative training to help the Primary Care Provider implement the care of patients. These tasks include:
  • Checking our patients in and out of their visits with the Provider
  • Registering patients, including gathering copies of insurance cards, driver's licenses or identification cards, authorizations, referrals and other medical records - managing the upload and routing documents in athena, our EMR
  • Scheduling patient appointments and updating patient information within athena
  • Working with patients and other physician offices to obtain medical records
  • Managing the phone line for the clinic - e.g., appointment reminder calls, refills, following up on referrals, and tracking of lab orders.
  • Completing patient intake, including vitals and history for clinical encounters
  • Venipuncture and completion of basic lab tests, e.g., UA, rapid strep/flu at the direction of the Provider
  • Removing sutures and changing dressings at the direction of the Provider
  • Administering medicine and injections - at the direction of the Provider
  • Preparing and assisting with medical procedures and exams
  • Perform Electrocardiograms (ECG) for providers to read
  • Providing basic patient education, e.g., diet, exercise, and providing iFOBT kits
  • Assisting with Population Health Management, e.g., patient outreach and panel list management
  • Going above and beyond in all circumstances to ensure members receive the best care
  • External vendor lab management, including but not limited to Quanum for ordering supplies and retrieval of lab results

You will support our Patient Advocacy and Community Team by:
  • Supporting with the completion of the PCP Conversion form - this is how patients become Matter Members
  • Being visible in the community by participating in community events
  • Providing Social Determinate of Health (SDOH) support as needed
  • Assisting in the coordination and execution of Member engagement activities leading up to the clinic launch to build deep and trusted relationships within the community
  • General tasks to help increase growth of Membership

Requirements
What Leads to Success?
  • Passion for Matter Health model, vision, and patient population
  • A Certified or Registered Medical Assistant or a State Licensed Practical Nurse
  • 3 years of clinical and administrative experience
  • 3 years of experience working in an Electronic Medical Record System
  • A graduate of an accredited nursing program
  • Basic Life Support (BLS) certification
  • Phlebotomy experience and certification required or willing to obtain in six months
  • Strong computer skills and a basic knowledge of Microsoft office
  • Ability to work without direct supervision and perform tasks autonomously
  • Ability to collaborate with professional colleagues as necessary to provide quality care
  • Ability to consistently present a friendly, welcoming, and professional public facing appearance
  • US Work Authorization Required

Nice to Haves:
  • Primary Care, Family Practice, or Geriatric Experience
  • Experience working with athena (EMR) preferred
