POSITION OVERVIEW

We have an opportunity for a motivated Care Manager to join our team and contribute to the delivery of quality services at The Commons! We recently have restructured our department to enhance the services we are able to provide to our residents which means we added positions and need more people to help our team succeed!
This full-time position has a schedule of 1:30pm -10:00pm. We have the option of Tuesday - Saturday or Sunday - Thursday schedules available. This position also participates in an on-call rotation.
This position has a starting wage range of $22.50 - $23.62 per hour.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Hiring, evaluating, disciplining and providing oversight and supervision to Care Specialists including coordinating duties.
- Conduct pre-admission assessments to create resident care plan.
- Conduct re-assessments for residents as their needs change and annually.
- Write and update care plans based on resident assessments and re-assessments.
- Communicate with Families and External Providers to ensure the needs of the resident and facility are met.
- Provide oversight and management of the levels of care and ensure appropriate fee charges are entered into appropriate software.
- Make appropriate referrals to external providers: i.e. Hospice, Home Care, Rehab.
- Schedule care specialist tasks in Vision and ensure care plans are up to date and accurate.
- Collaborating with Care Department Director and Nurse as needed to ensure residents are receiving the highest quality of care.
- Ensure appropriate documentation by care specialists.
- Manage complaint investigation and provide assistance during occurrence reporting process.
- Follow up on Medical Appointments and Coordination with the Medication Office to ensure health care provider's orders are being followed.
GENERAL DUTIES AND RESPONSIBILITIES
- Schedule includes on-call duties.
- Regular attendance/punctuality.
- Cooperate with others.
- Be completely honest.
- Follow up as needed with out of facility residents, providers, and family.
- Oversight and management of resident move-in process.
- Participating in: Care Conferences, CORE leadership meetings and Care Concerns meetings.
- Order and/or request resident supplies as needed.
- Cover/ provide direct care as needed.
- Complete Annual Safety Checks for residents to ensure compliance with Life Safety regulations and State regulations
- Be completely honest
- Other duties as assigned.

- Valid Colorado Driver's License (required)
- Proof of current automobile insurance (required)
- Qualified Medication Administration Personnel (QMAP) Certification (required)
- CPR Certificate/First Aid Certificate (required)
- De-Escalation Training (preferred)
- Hilltop's Transportation Certification (preferred)
