NOTE: This is a remote position, however, residence in *******Ability to accommodate Eastern or Central time zone is required.
We are looking for a highly motivated and energetic Care Manager to join our team. The ideal candidate for this role is a medical assistant who is experienced and comfortable with Remote Patient Monitoring and Principal Care Management (or Chronic Care Management) workflows. We focus heavily on coordinating care for our patients as well as improving patient outcomes in the post-acute space.
Responsibilities:
- Monitoring patient data and trends to formulate appropriate care plans
- Engage in conversations with patients/providers and post-acute stakeholders regarding clinical best practice recommendations
- Documentation of medical discussions (topics, goals and outcomes)
- Answer patient and provider questions regarding PCM and RPM services
- Explaining benefits of the program to the patient
- Conducting onboard and follow-up calls with patients
- Follow HIPAA guidelines and regulations
Requirements:
- Certified Medical Assistant OR qualifying medical certification
- High School diploma or equivalent
- Preferred: 2+ years of post-acute healthcare experience (Skilled Nursing Facility and/or Home Health Agency)
- Advanced computer skills
- Telephonic and electronic messaging communications skills
- Ability to work with multiple computer screens and programs simultaneously
- Positive attitude and ability to work well with others in a team environment
- Preferred: Previous Remote Patient Monitoring or PCM/CCM experience

Pay: $17.00 - $20.00 per hour
