Care Manager - Chronic Care Navigator

As a valued partner, Phamily is assisting Care Free Medical in filling their need for a CHRONIC CARE NAVIGATOR / MEDICAL ASSISTANT. This candidate must reside in the state of Michigan and is primarily remote, although some trainings and meetings in office are preferred. Strong candidates who are within the state of Michigan but not travelling distance to the office may be considered. 

The Chronic Care Navigator implements, manages, and coordinates all Chronic Care Management Program activities. You will directly engage, manage, and triage complex clinical needs.

YOU WILL BE RESPONSIBLE FOR:

● Developing a proper understanding of chronic pain and family medical care requirements for optimal, coordinated population health

● Proactive patient management via text using Phamily platform

● Proactive patient management via phone calls

● Triage and assessment of patient health status

● Coordination of care between patient and provider

● Active review of patient charts to assess health needs and gaps

● Providing a positive patient experience for patients and families through courteous digital interactions

● Routing patient escalations accurately and expeditiously

● Making referrals to appropriate staff when necessary

SALARY: $15.00 - $18.00 per hour

Opportunity to scale up to $19-21/hr upon performance evaluation.

POSITION HOURS: 9am-5pm (primarily)
