Requirements
Description of Role: Reporting to the Care Management Supervisor, this position is responsible for the overall care management of a diverse caseload. The Care Manager facilitates services and supports informal caregivers who care for an older adult, adult with a disability, or older relative raising child/ren living in the community. This position carries responsibility for ongoing person-centered care planning, service arrangement, initiating interventions and follow-up activities, and completing assessments. The Care Manager is a consumer advocate assisting with service access, information, and referral, identifying community supports, service provision, and monitoring consumer satisfaction.
Required Education: One year as an Aging Case Aide 2 or equivalent position. OR A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field. OR An equivalent combination of experience and training.
Job Responsibilities: To maintain a high quality of care management services by identifying and meeting the varied needs of PCA (Philadelphia Corporation for Aging) Caregiver Support Program (CSP) consumers. The Care Manager maintains the highest level of professional ethics, continually seeking opportunities for growth and development.
a. Performs all care management functions. Completes assessment of the consumer's and caregiver's needs in their environment. Reassess consumer's and caregiver's status every six months.
b. Works with consumer/caregiver, family, and care receiver, to develop a person-centered care plan, making use of supervisory consultation.
c. Recognizes, supports, develops, and mobilizes informal and formal resources to meet consumers' unmet needs.
d. Arranges for needed services, and entitlements working cooperatively with consumer, family members, and service providers. Follow up on services delivered and monitors consumer satisfaction.
e. Completes all necessary forms for consumer's case record, other management information, and other written reports as required. Enters inputs and necessary data and maintains automated programs.
f. Participates in orientation and training, in-service training as assigned, and attends regular team and staff meetings.
g. Monitors consumer satisfaction to ensure quality of services provided.
h. Effective management of a complex case mix and caseload.
i. Assists with community engagement, outreach, and education activities.
j. Maintains telephone at home visit contact requirements with caregivers.
k. Implements policy, procedures, and objectives of CSP as per program requirements.
l. Ability to work remote/hybrid including in-office work as scheduled.
m. Other duties as assigned.

- Meets agency standards of care management practice.
- Able to prioritize and complete assignments independently.
- Records and reports are legible, timely, accurate, complete, and relevant.
- Ability to establish rapport with PCA staff, consumers, providers, and outside agencies.
- Identifies problems and recommends strategies for intervention.
- Assures quality services to consumers.

- Must have valid driver's license, a safe driving record, and continuous access to a fully insured car.
