Position Summary:


Under the general supervision of the Specialty Supports Manager, the Care Management Utilization Supervisor position will provide global support to Care Management (CM) Leadership in the form of consultation and training around the utilization of appropriate member supports to promote achievement of outcomes for Lakeland Care's membership, specifically focused on the care planning and support needs for members with complex needs. The Utilization Supervisor will participate in all Utilization Management Committee meetings and relevant activities. This support will utilize organizational data, analyze utilization trends, and support Care Management practice teaching, training, and growing CM Supervisors’ knowledge base and ability to provide direct support to IDT staff in appropriate utilization of member specific supports and services.


Responsibilities & Competencies:

  • Provide day to day leadership and consultation with Lakeland Care’s Utilization positions to ensure the development of strong practice supports to inter-disciplinary staff and CM leaders.
  • Serve as a resource and collaborates through consultation with CM Supervisors to support comprehensive, outcome-based care planning for members with complex care planning needs, such as mental health support needs, behavioral health support needs, other challenging social or health determinants, etc.
  • Works in partnership with others to develop and maintain cost-effective service authorization practices, implement quality improvement initiatives, conduct utilization review, and review the effectiveness of service coordination policies and procedures to ensure effective delivery of the Family Care benefit.
  • Routinely interface with other internal Divisions to support collaborative provider growth and development to ensure fiscal sustainability and appropriateness of service utilization.
  • Uphold the guiding principles of the organization, lead by example, and hold each other and all staff accountable. Ensure direct reports also lead by example and are supportive to the various departments and most importantly the membership.
  • Gather and evaluate information that leads to sound, smart decisions.
  • Identify obstacles and address problems cooperatively as they occur.
  • Exhibit maturity, resiliency and sound judgement when dealing with organizational challenges.
  • Respond constructively to emotional situations, high pressure, and conflict.
  • Ensure adherence to DHS contract requirements through internal policy, procedure and best practice development and implementation.

Requirements:
  • Bachelor’s degree or higher in a Health or Human Services related field, or equivalent combination of formal education and experience (includes knowledge, skills, and abilities).
  • Minimum of three (3) year’s professional experience in a long-term care organization or community-based setting required; work experience with one or more of the target groups served by Family Care preferred.
  • Knowledge of community based long term care and service delivery for the elderly, physically and intellectually/developmentally disabled adults is preferred.
  • Strong data analysis and application skills.
  • Strong evaluation, assessment, and interview skills.
  • Strong oral and written communication skills.
  • Working knowledge of basic computer programs and typing skills.
  • Current driver’s license, acceptable driving record and proof of adequate insurance required.
