Position Summary:

The Care Management Senior Manager is responsible for operationalizing the strategic vision for the division that promotes excellence in Care Management (CM) practice and is aligned with the mission, vision, and values of Lakeland Care, Inc. (LCI). This role will oversee planning, organizing, and directing the activities of LCI’s interdisciplinary team staff (IDT staff) as well as be responsible for management of effective and cost-effective care planning that result in excellence in member outcomes and satisfaction. The Care Management Senior Manager ensures that collaboration occurs across departments to ensure best practice and preventive measures for membership are in place and followed. The role promotes consistency in practice and infuses innovation and continuous quality improvement efforts into exceeding member’s expectations.

Responsibilities & Competencies:

  • Ensure deliverables are met for the division, including strategic goals, outcomes and quality measures related to Annual Quality Review findings.
  • Identify obstacles and departments needed to resolve issues and ensure outcomes are met.
  • Assist in development and maintenance of departmental procedures and documentation.
  • Identify opportunities to promote LCI with various stakeholders, and in alignment with our philosophy, values, and initiatives.
  • Support and participate in performance excellence, continuous improvement, and strategic planning.
  • Monitor data and reporting to ensure cost-effective and high-quality care planning.
  • Collaborate with internal stakeholders to develop and advance data and analytics to drive practice enhancements.
  • Liaison with external stakeholders such as provider agencies, organizations, and the Department of Health Services to strengthen systems support of members.
  • Monitor the utilization of Medicaid funds used for community support services.
  • Exhibit maturity, resiliency, and sound judgement when dealing with organizational challenges.
  • Demonstrate an ability to influence others through positive reinforcement, teamwork, and content knowledge.
  • Promote individual, team, and organizational learning opportunities.

Requirements:
  • Bachelor’s degree in Human Services, Nursing, Social Work, Mental Health, other related field, or equivalent combination of education and experience related to Care Management services (includes knowledge, skills, and abilities).
  • Minimum of five (5) years of management experience is required. Including supervision of staff.
  • Knowledge of community based long term care and service delivery for the elderly, physically and intellectually/developmentally disabled adults. Knowledge of the disabilities associated with LCI target groups preferred.
  • Strong business acumen, critical thinking, and professional judgement.
  • Strong evaluation, assessment, and interview skills.
  • Strong oral and written communication skills.
  • Leadership qualities necessary to supervise and train staff.
  • Working knowledge of basic computer programs and typing skills.
  • Current driver’s license, acceptable driving record and proof of adequate insurance required.
