Job Description
•Explain to the client/responsible party the role/scope of services of the Medicaid Specialist while also explaining to the client/responsible party their rights and responsibilities.
•Complete the Medicaid application and work with client and support systems to gather required verifications
•Collaborate with local ADRC’s to ensure timely completion of the LTCFS, inform them of timing of financial Medicaid submission, and update regarding outstanding verifications
•Coordination with IM agency regarding submitting requested verifications
•Referral to other specialists when needed including funeral homes, financial planners, and/or elder law attorneys
•Provide client and family education on the Medicaid process in collaboration with the ADRC
•Other duties as assigned
QUALIFICATIONS AND EDUCATION REQUIREMENTS
Requirements: Bachelor’s Degree in Psychology, Social Work or related field. Associates Degree in related field and two years of related experience in leu of Bachelor's degree. Certified Case Manager certification or CMC preferred.
PREFERRED SKILLS
Thorough understanding of government policies and legislation regarding benefit options. Patient situation assessment skills, Knowledge of the healthcare system and levels of care, Patient advocacy skills, Knowledge of the needs of the frail elderly, disabled and/or cognitively impaired client geriatric population. Experience in performing home visits. Experience with case management. Knowledge of community resources. Ability to assess cognitive, functional and social needs of the clients. Ability to interact positively, creatively, and professionally with others. Computer skills (Outlook, MSWord, Internet Explorer)
