Position Summary:

The Community Supports Manager will lead development and delivery of specialized services, supports, and providers for Lakeland Care’s (LCI) membership related to employment, housing, technology supports, community connections, and self-determination. This position will focus on LCI’s vision of person directed care planning to support the member’s engagement within their community by building strategies, supports, and providers for teams in care planning with the members. The Community Supports Manager will directly supervises the Community Connections Support Specialist, Community Supports Supervisor, and Self Determination Supervisor.

Responsibilities & Competencies:

  • Support care planning practice by promoting appropriate community living supports and processes for membership.
  • Collaborate with internal stakeholders to develop and advance data and analytics to drive practice enhancements.
  • Continue development and evolution of LCI’s self-determination philosophy and associated infrastructure to support increased utilization of self-directed supports (SDS) for LCI’s membership.
  • Creative problem solving and flexibility in Rate Negotiations, Service Provision and Contracting.
  • Provide education to contracted providers regarding the Family Care program and state statutes.
  • Liaison with external stakeholders such as provider agencies, organizations, and the Department of Health Services to strengthen systems support of members.
  • Strong Customer Service focus and timely communication with all stakeholders.
  • Monitor the utilization of Medicaid funds used for community support services.
  • Monitor effectiveness of the Pay for Performance initiatives within LCI and adjust as needed with any changes including, but not limited to, implementing strategies and interventions to achieve desired outcomes.
  • Exhibit maturity, resiliency and sound judgement when dealing with organizational challenges.
  • Demonstrate emotions appropriate to the situation and continue to perform steadily and effectively.
  • Promote individual, team, and organizational learning opportunities.

**Hiring anywhere within Wisconsin only. Ideal candidate will be local in the Central and Northeastern regions to support staff and members.

**Travel will be required as needed.

Requirements:
  • Bachelor’s degree in Human Services, Nursing, Social Work, Mental Health, other Community Support related field, or equivalent combination of education and experience related to Community Support services (includes knowledge, skills, and abilities).
  • Minimum of one year of supervisory/leadership experience is required, two or more years preferred.
  • Minimum of five (5) year’s professional experience in a long-term care organization or community-based services required with no less than one (1) of those years being directly related to Community Services and/or Self Determination.
  • Knowledge of community based long term care and service delivery for the elderly, physically and intellectually/developmentally disabled adults. Knowledge of the disabilities associated with LCI target groups preferred.
  • Strong business acumen, critical thinking, and professional judgement.
  • Strong evaluation, assessment, and interview skills.
  • Strong oral and written communication skills.
  • Leadership qualities necessary to supervise and train staff.
  • Working knowledge of basic computer programs and typing skills.
  • Current driver’s license, acceptable driving record and proof of adequate insurance required.
