Responsibilities:
- Serve as liaison, coordinate, monitor, evaluate and reevaluate clients’ status and their care. This includes making and monitoring medical appointments, documenting medical and personal information and coordinating with Fiduciary.
- Monitor clients’ health and welfare through personal visits at their home or facility, interaction with staff, family, friends and anyone who assists with the clients’ lives.
- Communicate client needs to the lead fiduciary, doctors, nurses, other medical providers and other care team members and follow up accordingly.
- Act as the client’s advocate as it relates to all services and assistance they may require.
- Maintain thorough records through detailed documentation in Teams.
- Coordinate with vendors as necessary to ensure all continuity of all services for clients, from medically related needs to personal needs such as ordering personal items, utilities, and any other services the client may require.
- Must have valid a vehicle, valid driver’s license and insurance
- CNA certificate required
Job Type: Part-time
Pay: $22.00 - $26.00 per hour
Expected hours: 10 – 20 per week
