Columbia Community Mental Health is a nonprofit agency and Certified Community Behavioral Health Clinic that provides services to individuals with mental, addiction and developmental disabilities. Columbia Community Mental Health houses the Collaborative Care Team, an approach to wellness that provides clients with therapy, case management, peer support, and addictions counseling as appropriate. We are looking for a qualified Care Coordinator to join our team!


Position:Care Coordinator

Shift: Mon-Fri 8:00 am to 5:00 pm

Salary Range: $26.01/hour - $33.63/hour. We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements).

Position Requirements: requires a QMHA: a bachelor's degree in a behavioral science field from an accredited college or university or three years of full-time professional experience working in the mental health related field. Note: All QMHA positions in the state of Oregon will need to be filled by candidates who are officially QMHA certified or registered with MHACBO. For details, go directly to MHACBO (www.mhacbo.org)

Summary: The Care Coordinator functions collaboratively as part of our Collaborative Care Team. The CCT is comprised of a multidisciplinary Behavioral Health team. This team addresses Behavioral Health needs as well as case management needs. The Care Coordinator will provide compassionate, person-centered, trauma-informed care to individuals with the goal of assisting with needs involving housing, food instability, medical rides, and other community resources as needed. An ideal candidate to fill this Care Coordinator position will work with community partners, stakeholders, and the multidisciplinary team within the Collaborative Care Team to coordinate client care and safety. The successful candidate will have experience in working with individuals with psychiatric diagnoses or symptoms.

Working conditions: This position works in an office environment as well as in the community, with about 60% of the work done in the community. This position requires driving to different locations within Columbia County and some occasional driving to locations elsewhere in the Portland Metro. The Care Coordinator will also be available to provide services in the client's residence as needed.
