Care Coordinator LPN This LPN position will serve to support both existing Triage and Case Manager functions. Triage-Serves as the first clinical contact for a patient seeking advice or treatment for an acute condition. The Triage Nurse provides health advice to patients over the phone while performing an evaluation of patient symptoms and provides recommendations for escalating treatment. The Triage Nurse follows established procedures and protocols, and documents calls as required to ensure the safe, effective and appropriate disposition of health related problems. Case Management-Actively participates with patients to identify and facilitate options and services for meeting their individual health needs, with the goal of decreasing fragmentation and duplication of care and enhancing quality and cost-effective clinical outcomes. Assists RN Case Manager with developing, implementing, and evaluating individualized patient care plans with the healthcare team, and serves as a liaison for working with patients and their families. .

    1. Triage 1.1. Answers and manages nurse-directed patient calls. Coordinates same-day and walk-in appointments when deemed appropriate. 1.2. Evaluates and assesses patients health care needs based on patients signs and symptoms utilizing established protocols and guidelines. 1.3. Obtains appropriate input and direction from the health care provider to determine the best clinical course of action and recommendations for specific patient health care needs. 1.4. Provides clear patient information and education; and provides clinical advice within the scope of practice on handling patient's situations. Ensures consistent and effective health care outcomes of patients and their access to appropriate health care. 1.5. Documents in the EMR triage assessment, plan, implementation, and follow-up. Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. 1.6. Manages refills both electronic and faxed requests from the Pharmacy. Review EMR for appropriateness of request before forwarding electronically to provider for authorization. 1.7. Assists providers with coordination of MyChart patient messaging. 1.8. Performs other nursing functions as assigned. 1.9. Participates in performance improvement activities as assigned.
    2. Case Managerment 2.1. Works with the entire health care team, including collaboration with providers, social work and patient navigators. 2.2. Reviews all incoming Specialty Care Referrals. Prioritizing stat and overbook requests and declining incomplete or inappropriate referrals. 2.3. Assist in obtaining Specialty Care and drug prior authorizations for BH PCC Primary Care patients and relay such authorizations (or denials) to patients, providers and pharmacies. 2.4. Assist in appeals and peer-to-peer process with insurance carriers for BH PCC Primary Care patients with denied advanced imaging requests. 2.5. Attempt to resolve all Specialty Care Service and denied advanced imaging requests prior to the patients appointment. 2.6. Assists in implementation of patient-centered plan of care including self-management goals and care plans. 2.7. Documents case management plans and on-going activities in EMR.
    3. Financial/Volume Results 3.1. Assists staff in collection of quality, utilization and satisfaction data. 3.2. Provides input into the design and the implementation of monitoring and evaluation strategies and tools as indicated by feedback from supervising RN or manager, physicians/doctors.
    4. Other 4.1 Assist in maintaining PCMH Certification 4.2. Collaborates with all members of Healthcare Team to assure PCMH principles are consistently implemented.
Qualifications:

EDUCATION


EDUCATION: Graduation from an accredited Practical Nursing program


EXPERIENCE


EXPERIENCE: 3-5 years clinical experience in the area of specialty. Strong interpersonal and organizational skills.


LICENSURE


Current LPN licensure in the State of Connecticut


SPECIAL SKILLS


SPECIAL SKILLS: EPIC experience preferred, proficiency in Excel
