SalaryPremier Health Associates is Sussex County’s largest provider of health care in the region. As our multi-specialty practice continues to grow, we are seeking additional motivated and engaging Medical Assistants to join our team as Care Coordinators. We are looking for a medical professional with 3+ years of clinical experience and a solid understanding of chronic conditions and care plan development. Care Coordinator candidates must possess excellent clinical, organizational, and time management skills with ability to work independently and collaboratively with physicians and support staff. Knowledge of value-based concepts, population health informatics and strong computer literacy are beneficial. This position in a non-client facing position, all patient care is telephonic.
