Care Coordinator:

The Care Coordinator is responsible for the delivery of patient care services through coordination, implementation, and supervision of patients.

Position Summary:

With a Quality, Customer First, and Compassionate approach, The Care Coordinator will:

  • Identify clients that will benefit from care coordination. Work with agencies and service providers to facilitate care coordination and information sharing.
  • Provide case management to assigned patients.
  • Team Based Care – Clinical Care Collaboration – Work in collaboration and continuous partnership with providers, clinical staff, and community resources in a team approach to promote access to appropriate care in the goal of enhancing patient health.
  • Pre-Visit Team Based Planning – Review daily schedule and chart by care team. Documented instructions are written on the schedule, typed into the EHR appointment note, or in the EHR as a flag or clinical list update. Ensure patient visit is more efficient by identifying: 1. Records from transitions of care are available during the patient visit. 2. Results from labs or diagnostic are in the EHR. 3. Labs or other services that are not specified in the standing orders protocols.
  • Screen patient for healthcare needs.
  • Develop a client-focused case management plan.
  • Document patients’ case management plans and ongoing activities. Chart data in Electronic Health Record (EHR).
  • Identify patients’ insurance coverage or other sources of payment for services.
  • Identify and address patient risk factors and/or obstacles to care.
  • Identify patient needs, current services, and available resources and connect the patient to services and resources to meet established goals.
  • Communicate the care preferences of patient, serving as their advocate, and verifying that interventions meet the patients’ needs and goals of treatment.
  • Educate the patient/family/caregiver about the case management process and evaluate their understanding of the process.
  • Monitor patients according to the guidelines set by the CDC, CCGC, State and Sunrise Community Health.
  • Assist with scheduling appointment, follow-up care, referrals and medication refills.
  • Attend assigned trainings (cardiovascular, CCSP, PM, Medicaid).
  • Provide health coaching and patient education for Diabetes patients.
  • Connect patients with MFC for colonoscopy appointments.
  • In charge of tracking and distribution of insulin samples.
  • Back up to Social Worker.
  • Newborn coordinator back-up.
  • Assist patients with transportation needs (Bus Tokens).
  • Other duties as assigned.


Minimum Qualifications:

  • High School Diploma or GED.
  • Bachelor’s Degree (B.A.) in human services, social work and/or nursing preferred.
  • 1-2 years related experience and/or training, or the equivalent combination of education and experience.
  • Bilingual English/Spanish Preferred
