A&B Homecare Solutions, a Modivcare Personal Care Service, is looking for a Care Coordinator to schedule patient visits with a caregiver, document communication between patients and caregivers, and ensure branch compliance with patient-related documentation requirements. You will report to the Area Director or local Branch Manager.

• Pay: $19.00-$23.00/Hourly
• Schedule: M-F 8:30AM-5:00PM

You will...

• Serve as the primary point of contact for patients, families, and healthcare professionals regarding care coordination.
• Work with Field Supervisors and Case Managers to coordinate appropriate staff placements for clients.
• Address inquiries, concerns, and provide guidance on accessing services and resources.
• Monitor and update schedules to accommodate changes, cancelations, and rescheduling needs.
• Communicate pertinent patient information, including diagnosis, treatment plans, and care instructions, to the appropriate individuals.
• Orient employees to new cases, thoroughly covering all needs of the client to the caregiver assigned to the case.
• Maintain accurate and up-to-date electronic and physical records of patient information, appointments, and communication.
• Provide compassionate and empathetic support to patients and their families throughout the care coordination process.

• High school diploma or equivalent
• 1+ years of Customer Service experience required.
• 1+ years of Home Care experience preferred.
