Description
Title:Care Coordination Program Manager
Reports to: Assistant Director of Care Coordination Services
Job Objectives:
Under the direction of the Assistant Director of Care Coordination Services, assist with the oversight of Mental Health Association in Orange County Inc.'s (MHA) Care Management Department and ensure the success of the program.
Perform duties while actively promoting an environment consistent with MHA's Cultural Equity Policy and WELCOME Orange.
Responsibilities:
• Attend bi-weekly Care Coordination team meetings.
• Attend bi-weekly Supervisor team meetings.
• Assist in training Supervisory staff and new care management staff.
• Quality Assurance duties:
Responsible for completing required data collection, statistical, and program reports including but not limited to audits, scorecards, gaps in care, transitions of care, HARP/HCBS, and HEDIS measures.
• Review and ensure all Health Home program participants' files adhere to compliance guidelines from funding sources and other applicable regulatory agencies (i.e. Medicaid, NYS Office of Mental Health, NYS Department of Health, HIPAA- US Department of Health and Human Services).
• Along with Program Manager(s), Supervisors, and Assistant Director, ensure program's compliance with Medicaid billing standards.
• Along with Program Manager(s), Assistant Director, and Quality Assurance/Administrative Manager, ensure program is routinely evaluated for the purposes of developing and implementing quality improvement plans as needed.
• Responsible for the direct supervision of select Care Management staff.
• Completes introductory period reviews and annual performance reviews/evaluations for supervised staff.
• Responsible for the general oversight of the Care Coordination Program along side Care Coordination Program Manager(s) and/or Assistant Director's absence.
• Complete New York State Community Mental Health (CMH) assessments and reassessments as required.
• Assists in the oversight of the outreach process of all Health Home referrals including but not limited to: sending information letters, phone contacts, street contacts, researching referral locations and maintaining referral lists by outreach and hiatus status.
• Represent the agency at the weekly Single Point of Access (SPOA) meetings, as needed.
• Attend trainings and meetings relevant to Health Homes.
• Ensure that all client records are maintained electronically through FCM and AWARDS.
• Assist in oversight of new client intakes and ensure referrals are entered in FCM and AWARDS, as needed.
• Coordinate monthly billing for outreaches and assigned caseloads.
• With assistance and guidance of Assistant Director and Program Manager(s), help to assign and monitor caseloads and determine appropriate level of caseload.
• Assist in reviewing and approving all assessments, reassessments and care plans.
• Collaborate with Assistant Director and Program Manager(s) to discuss and approve/deny service dollar requests and ensure all service dollar forms and receipts are completed correctly, kept track of, and submitted to the finance department in a timely manner.
• Establish and maintain linkage with appropriate community agencies and service providers.
• Represent MHA at meetings and events as necessary.
Other Duties:
• In the spirit of teamwork, answer MHA's main office phone as needed.
• Participate in MHA's committees as needed.

Requirements
IQualifications -Education:
• Bachelor's Degree in any of the following: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing;
OR
NYS licensure and current registration as a Registered Nurse and a bachelor's degree;
OR
Bachelor's level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses;
OR
A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
Qualifications - Experience:
• Two years of supervisory experience required.
• Two years of experience (a Master's degree in a related field may substitute for one year's experience) either:
o Providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance use.
OR
o Linking people who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse to a broad range of services essential to successfully living in a community setting.
• Bilingual Spanish / English a plus.
• Effective written and oral communication skills, including public speaking ability and experience.
• Excellent interpersonal skills.
• Excellent tact and diplomacy skills.
• Excellent attention to detail.
• Proven record of excellent analytical, planning and organizational skills.
• Exhibit proficiency with MS Office suite (Word, Excel and PowerPoint).
• Experience with Health Homes highly preferred.
• Knowledge of Medicaid procedures highly preferred.
• Experience with Electronic Health Record Program(s) required.
• Ability to work independently with minimal direct supervision.
• Must be self-motivated and have the ability to coordinate multiple tasks simultaneously.
• Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.
• Valid Driver's License and ability to drive with current driving record acceptable for agency insurance coverage.
Terms of employment/benefits:
- Exempt.
- Forty (40) hours per week.

Salary Description
$60,000 a year
