Care Associates role is to provide care and assistance to patients who have chronic illnesses, injuries, disabilities, or age-related challenges inside the patient's home. They monitor the patient's condition and help them in any activity they may need assistance in. They provide support to both the patient and the family to manage the patient's condition. Home health aides care for the patient by providing personal services like bathing and basic grooming, designing nutritional plans, cooking, and preparing meals, running errands, maintaining the cleanliness of the home, managing medication intake, and ensuring the home is safe and secure for the patient.
Job Responsibilities (varies):
The responsibilities of a Care Associate may include, but are not limited to the following:
● Recording pertinent information pertaining to the client’s care.
● Reminding clients to take medication at the appropriate time.
● Planning and preparing meals according to specific dietary requirements of the client, and if necessary, feeding the client.
● Moderately assist clients with personal care including hair care, oral hygiene, bathing, grooming, and dressing.
● Assisting with ambulation with or without mechanical aids.
● Assisting with routine bodily functions: Example, toileting.
● Maintaining a safe, clean and healthy environment through light housekeeping including changing bed Linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry.
●Providing companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments.
● Performing other housekeeping tasks as indicated in the care plan.
● Observing and reporting changes in client's condition to the Care Manager.
● Accurately preparing daily records and submitting them to the office by date due.
● Participating in in-service education programs.
●Other duties may be assigned
Education
High school diploma or equivalent
General Requirements
● A Care Associate must have a minimum of one year of personal care experience which is verifiable through references and must complete all steps in the employment process.
● Must be at least 18 years of age.
● Must be physically able to perform the duties of the position.
● Must exhibit mature responsible behavior and understand the need for patient confidentiality.
● Must be able to follow direction and accurately report to the Care Manager any changes in client's condition.
● Must have available reliable transportation to and from assignments.
● Must have good personal hygiene and organizational habits.
Work Environment/Additional Requirements
• Job is typically performed in patients' home
• Some general office hours
• Travel will be required
• Valid Driver’s License
• Strong written and verbal communication skills
