The Call Center Specialist, working as part of the Hospital Follow-Up Program, will make follow-up calls, perform crisis screening and provide referral/information services to individuals who have been identifies as possibly being at risk for suicide. Assists in the delivery of crisis intervention services to children and adults in a mental health crisis; coordinates the utilization of other County funded crisis and emergency services; links new clients with appropriate counseling and community support program services.
- Monitor the referrals from hospitals and other referral sources who are being released from a hospital setting and who have consented to receive follow-up calls to check on their clinical status
- Conducts screening interviews by phone and/or by crisis chat or text to provide emotional support and determine the most appropriate service(s) and available resources.
- Ensure that follow-up calls and services are made and service documentation has been completed, and to prepare and submit process and outcome reports.
Bachelor’s degree in social work, psychology, sociology or related field, and one year of professionally supervised work in the delivery of mental health, chemical dependency and/or social services
- Flexible schedule, which includes some evening and/or weekend hours.
- This is a remote position with some in person training and meetings required.
This is non-exempt, union position with a minimum pay rate of $18.00 per hour.
