JOB SUMMARY: Responsible for coordinating monthly visits with provider and residential care facilities.
ACCOUNTABLE TO: Call Center Scheduling Lead
JOB DUTIES:
Responsible for coordinating monthly visits with provider and residential care facilities in a caring and professional manner:
1. Responsible for scheduling monthly visits for patients at the 550 plus residential care homes for various services such as but not limited to medical, psychiatry, podiatry, & dental services.
2. Answers phones, screen and triage incoming calls, and direct calls appropriately.
3. Responsible for scheduling medical necessity visits including hospital follow up, quality measures, annual physical, & vaccine visits.
4. Contacts facilities to offer follow-up for any missed appointments.
5. Calls facilities to remind them of appointment the day before.
6. Schedule and coordinate mobile unit visits at residential care facilities.
7. Responsible for updating the residential care master schedule and facility list with daily updates.
8. Assist residential care facility administrators with scheduling clinic appointments.
9. Resolve issues involving CCCHC that arise in their assigned residential care facilities.
10. Develop strong relationships with providers via telephone and establish strong relationships with team members.
11. Provide an effective communication link between medical staff, behavioral health staff, and residential care facilities.
12. Assist in collecting patient documentation for new patients.
13. Create telephone encounters.
14. Provide customer service to 550 plus residential care facilities and to assigned providers.
15. Attend all required meetings and trainings.
16. Consistently demonstrate and uphold CCCHC’s principle of providing quality health and human services to the medically underserved and low-income populations in a culturally sensitive manner.
17. All other duties as assigned.

Education and Experience Requirements
- High School Diploma or GED required.
- Bilingual in English/Spanish required. (Other languages as necessary)
