The Position:


The Bleeding Disorders Clinical Transition Coordinator will work directly with the Bleeding Disorders community, patient advocacy groups, prescribers, patients, manufacturers, and company resources consisting of bleeding disorder territory managers, payor relations, and Bleeding Disorders Therapeutic Care Management Center (BDTCMC) team of patient access, reimbursement, and patient care operations team members to coordinate new starts, therapy optimization, product education, and adherence management. In addition to providing a significant level of patient and family education, this function will be responsible to educate referral sources regarding Soleo Health’s Bleeding Disorders services in collaboration with the Bleeding Disorders territory managers and local operations managers. The position will assist patients in their transfer to Soleo Health after the patient has selected the company as their provider of choice. Responsibilities include:

  • HTC/Healthcare Provider Education: Work with the operations manager and sales team to educate referral sources about the Company’s Bleeding Disorders services in a geographic market. Work collaboratively with the local Bleeding Disorders territory manager to support new referrals from hemophilia treatment centers and other healthcare providers. Promotes the company internally and externally while assisting in the development of strategic planning initiatives.
  • Patient Coordination: Works closely with multidisciplinary team to maintain and implement up-to-date coordinated care plan to transition patient’s care to Soleo Health; communicates with all members of the healthcare team on behalf of the patient. Provides feedback to healthcare providers regarding the acceptance and progress of patients and documents all demographic, clinical, and payer information in collaboration with the BDTCMC teams and Bleeding Disorders territory managers. Assists in securing necessary paperwork and signatures needed to commence therapy and submit for payer reimbursement, as necessary. Assists in patient education to address product education, therapy optimization and adherence management in collaboration with BDTCMC team.
  • Nurse Liaison: Communicates with referring treatment center’s/provider’s office as appropriate to the patient and physician’s needs. Works as a liaison between the company and healthcare providers, including discussing the benefits of Soleo Health’s Bleeding Disorders services and clinical outcomes.
  • Patient/Family Education: Conducts patient visits to introduce patient and family to Soleo Health services; introduces the company upon acceptance of a healthcare provider’s referral and explains available Bleeding Disorders services and financial/payer responsibilities to patient in coordination with Bleeding Disorders Patient Access team members. Works with marketing and sales team to educate patients and their families about available resources in the community for Bleeding Disorders patients and caregivers. Serves as a patient advocate and conduit to the company.
  • Administrative: Maintains accurate, and submits on a timely basis: expense reports, referral contacts/education tracking, mileage reports, territory reports, business development plans and other documents and training programs mandated by the company, as appropriate.




Requirements:
  • Currently Licensed Registered Nurse or Nurse Practitioner, bachelor’s degree preferred.
  • Current Clinical License
  • Minimum five (5) years of clinical experience, home health field or management experience preferred.
  • Knowledge of Bleeding Disorders patient and caregiver health care, financial and social services needs and experiences.
  • Knowledge of Bleeding Disorders health care providers processes and requirements for ongoing care for Bleeding Disorders patients.
  • Advanced practice expertise to identify and implement improvement processes, and the ability to design, direct and implement health care plans on behalf of the patient.
