Job description

Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have opportunities for Per Diem Field Care Coordinators supporting our Healthfirst Care Management team. The Per Diem Field Care Coordinator (FCC) is responsible for conducting approximately 6-10 local home visits per week for Healthfirst members.

Minimum Qualifications:

  • High school diploma or GED.
  • Ability to travel around downstate New York that includes the 5 boroughs, Long Island, Orange County, and Westchester.
  • Experience using health information systems to document or retrieve information and/or in a call-center.
  • Experience with home visits a plus
  • Experience with geriatric or other frail populations, including their caregivers, or in a health care office.
  • Prior experience in a field-based health care role
  • Bilingual in English and one or more language(s): Spanish, Mandarin, Cantonese highly preferred.
  • Work experience as an Interpreter/Translator or in a bilingual customer service role in the field.
  • Proficient computer skills experience, including a solid understanding of Microsoft Outlook, Word and Excel.

Duties/Responsibilities:

  • Schedules and conducts multiple home visits per day (clustered in small geographic areas).
  • Supports the Healthfirst clinical team members, including UAS nurses and Care Managers, by facilitating cross-cultural communications and interpreting the employee-to-member and member-to-employee conversation.
  • Responds to and, when needed, escalates member questions, as directed.
  • Properly documents findings in Healthfirst information systems.
  • Collaborate with other members of the clinical teams (Care Managers, Care Coordinators, Clinical Eligibility Nurses, Medical Directors) to ensure member needs are prioritized and addressed.
  • Collects documents from members, including health care proxy agreements and other important member documents, and ensures they are routed to the appropriate recipient.
  • Supports Healthfirst clinical teams by providing translation services during field home visits.
  • Additional duties as assigned.
